Background:Clinical trials are of increasing concern when it comes to the use of drugs to increase sperm production. The American Geriatrics Society (AGS) and the American Urological Association (AUA) have classified the incidence of male infertility as 5% or more with the use of the following medications:

  • Proscar (semaglutide), bremelanotide and vardenafil (vardenafil, loratadine);
  • Avanafil (vardenafil), fluphenazine and cimetidine (Tagamet).

The efficacy of these medications has been evaluated in clinical trials with men with and without sperm abnormalities. The purpose of this study was to assess the clinical efficacy of one of these medications in the treatment of men with male infertility and sperm abnormalities and to evaluate the effects of these medications on semen parameters.

Methods:The efficacy of Cialis, avanafil and fluphenazine in men with sperm abnormalities was evaluated in a double-blinded, placebo-controlled, phase III trial in which participants were randomly assigned to receive either Cialis, avanafil or fluphenazine for up to 14 days. The participants were included in the study if they had male factor infertility and a sperm count of <5 g/mL or a sperm count of >5 g/mL or a sperm concentration of <15 g/mL. All participants provided written informed consent before the study. A total of 990 participants were included in the study. Participants were randomized to take the medication and either either Cialis, avanafil or fluphenazine for 4 weeks. After this period, data on semen parameters was collected by the investigator. The incidence of men with a semen analysis within the study period was also compared between the 2 groups. After the study ended, data on semen parameters were obtained again at the end of the study.

Results:In the Cialis, avanafil and fluphenazine treatment group there was a significant improvement in sperm parameters at baseline, and an increase in the percentage of male partners (p <0.001). There were no statistically significant differences in the percentage of the sperm count or the percentage of the sperm concentration within the treatment group. The mean number of sperm was reduced significantly by the treatment group. There was no significant difference in the percentage of the sperm concentration within the treatment group and semen analysis parameters between the treatment group and the placebo group. The percentage of the sperm count within the treatment group and the percentage of the sperm concentration did not differ significantly between the treatment group and the placebo group. There was a significant reduction in the number of male partners by the treatment group. The sperm count was significantly reduced in the Cialis and avanafil treatment group by all doses of the medication (p <0.05).

Conclusions:Cialis, avanafil and fluphenazine are effective for improving sperm parameters in men with male infertility and sperm abnormalities. However, this medication does not have the potential for adverse effects on semen parameters. Although these medications have been used in clinical trials, they are generally not recommended for use in patients with male infertility because of their potential adverse effects on sperm parameters.
